*In order to provide our patients with the most accurate information, we make great effort to keep this page current. However, these fees can and will change from time to time. *It is very common for patients to elect for multiple procedures to be performed at the same day of surgery. As a result of hiring the anesthesia provider only once for the case, there will be a cost savings for each additional procedure performed. There are limitations to the amount of surgery that can safely be performed at one session. Discuss these issues with your physician.

Financing Options

 1. We accept all major credit cards in addition to cash, personal check, cashiers check, and money order.
2. Financing Options - All are independent of this office and applications can be made immediately over the Internet in some cases. All are based on credit report findings and are subject to review before approval.  

 A.  Care Credit Finance - 1-800-365-8295.
CareCredit is a card card exclusive for healthcare services.  With CareCredit, you get a number of fixed low interest plans that include periods of 24, 36, 48 and 60 months. Click here to apply online for an immediate decision. * Our office does not participate in the no interest plan and there is a 5% fee to use Care Credit.

 B. Surgeryloans.com - Surgeryloans.com offers an extensive array of installment plan options with interest rates ranging from 5.9% to18.9% APR, payment plans up to 60 months, no pre-payment penalties, and loans from $1,000 to $25,000. C. Chase Health Advance - 1-888-519-6111
Chase Health Advance offers flexible payment plans of 3, 6, 12, 18, and 24 months or an extended payment plan up to 48 months.  Click here to apply through their secure site for an immediate decision.  *Provider Merchant ID Number - 13340. There is a 5% fee to use Chase Health Advance.

Unless we are currently running a special with a specific deadline any fees given to you on a patient quote will be honored for 90 days from the date the quote was given to you. 

 A non-refundable $500.00 deposit is required to schedule a surgical procedure.  The remaining balance will be due, in full, 1 week prior to your scheduled procedure.  Fees may be paid by cash, check, Visa, Mastercard, American Express, Discover, or through an approved finance company.  If you choose to pay with a credit or debit card (including surgical loan credit accounts), you will be charged a 2.5% transaction fee.  A 5% transaction fee will be added to payments with Care Credit or Chase Health Advance.

If you decide to cancel surgery for any reason without rescheduling at the time of cancellation, all deposits made toward surgery are non-refundable.  If you cancel without at least 14 days notice you will be required to re-pay the $500 deposit in order to reschedule.
