Financing Options
Payment for cosmetic surgery is due in full one week prior to your procedure, unless paying with a personal check, which must be received by Cosmetic Surgery Affiliates 14 days prior to your surgery. We provide a number of payment options that may be used individually or combined according to your wishes.
- Cash or check: If you are bringing cash, please make sure it is the exact amount. We do not keep cash on hand in order to make change. Please make all forms of checks out to “CSA” or “Cosmetic Surgery Affiliates.”
- Credit card: If you choose to pay with a credit card, note that a 2.5% transaction fee will be applied to the charge amount.
- Optional financing: We will be happy to assist you with applying for financing should you so desire.
Insurance Coverage
We are not affiliated with any insurance companies or plans and we do not accept insurance payments for services. Payment is due in full before any procedure will be completed.
CareCredit

CareCredit is a card card exclusive for healthcare services. With CareCredit, you get a number of fixed low interest plans that include periods of 24, 36, 48 and 60 months. Click here to apply online for an immediate decision.
*Our office participates in the no interest plan. We select the extended payment plan for you, and there is a scaled fee, based on your procedure price, to use Care Credit.
Chase Health Advance
Click here to apply through their secure site for an immediate decision. *Provider Merchant ID Number - 13340. There is a 5% fee to use Chase Health Advance. Our office only participates in the extended payment of 48 months.
Unless we are currently running a special with a specific deadline any fees given to you on a patient quote will be honored for 90 days from the date the quote was given to you.
Scheduling Your Procedure
A non-refundable $500 deposit is required to schedule a surgical procedure. The remaining balance will be due, in full, 1 week prior to your scheduled procedure.

- An approved finance company.
If you choose to pay with a credit or debit card (including surgical loan credit accounts), you will be charged a 2.5% transaction fee. A 5% transaction fee will be added to payments with Care Credit or Chase Health Advance.
If you decide to cancel surgery for any reason without rescheduling at the time of cancellation, all deposits made toward surgery are non-refundable. If you cancel without at least 14 days notice you will be required to re-pay the $500 deposit in order to reschedule.
